- Simplify the process by assisting with the application and necessary documentation
- Coordinate care services with professional, quality caregivers
- Assist in maintaining eligibility in the program
- Provide a “one stop shop” to answer any questions and ease the process
VA Health Care System – Community Care Network
If a Veteran is in the VA Health Care System and has needs for assistance with Activities of Daily Living (ADLs) and/or Instrumental Activities of Daily Living (IADLs) the Veteran can inquire with their VA Primary Care Provider (PCP) to see if the Veteran might be eligible (based on needs) to receive an order for non-medical services via their PCP for Home Health Aide / Homemaker / Respite services funded by the VA through their Community Care Network (CCN). If approved for Home Health Aide / Homemaker / Respite services, then you will be assigned a provider. You can specify your provider of choice. *The US Department of Veterans Affairs has a pension program for Veterans and surviving spouses with non-service related medical conditions. Eligible candidates may also receive Aid & Attendance benefits for reimbursement of non-medical home care services.
